Required Immunization

 

IMMUNIZATION REQUIREMENTS

Immunization Requirements that must

 be on file with school nurse before
 
starting school are as follows:

 

Pre–K:
If your child is 3 years of age:

  • At least three (3) doses of oral polio are required.
  • At least four (4) doses of DTaP vaccine are required.
  • One (1) dose of MMR (measles, mumps & rubella) vaccine is required.  It must have been received on or after the 1st birthday.
  • Three (3) doses of Hepatitis B vaccine.
  • Two (2) doses of Hepatitis A, the first dose must have been received on or after the 1st birthday.
  • One (1) dose of Varicella (Chickenpox) Vaccine on or after the 1st birthday.  Or a parent- or physician-validated history of Varicella illness (chickenpox).  If your child has had Chickenpox, please ask for a form that will be necessary to have on file.  
  • Three (3) doses of Hib vaccine with a booster after 12 months of age –OR- one (1) dose of Hib vaccine on or after 15 months of age.
  • Four (4) doses Pneumococcal (PCV-7 or PCV-13) or Prevnar –OR- three (3) doses  with one dose on or after 12 months of age –OR- two doses with both     doses on or after 12 months of age, or one dose on or after 24 months of age.
  • If your child is 4 years of age:
  • At least three-four (3-4) doses of oral polio are required, provided at least one dose has been received on or after the fourth birthday.
    • At least four-five (4-5) doses of DTaP vaccine are required, provided at least one dose has been received on or after the fourth birthday.
    • Two (2) doses of MMR (measles, mumps & rubella) vaccine are required.  Both doses must have been received on or after the 1st birthday.
    • Two (2) doses of Varicella (Chickenpox) Vaccine.  Both doses must have been received on or after the 1st birthday.  Or a parent- or physician-validated history of Varicella illness (chickenpox).  If your child has had Chickenpox, please ask for a form that will be necessary to have on file.
    • Three (3) doses of Hepatitis B vaccine.
    • Two (2) doses of Hepatitis A, the first dose must have been received on or after the 1st birthday.
  • Three (3) doses of Hib vaccine with a booster after 12 months of age –OR- One (1) dose of Hib vaccine on or after 15 months of age.

Kindergarten:

 

  • At least three-four (3-4) doses of oral polio are required, provided at least one dose has been received on or after the fourth birthday.

You may obtain these immunizations at the following:

 

  • Your family physician
  • Hardin County Health Unit in Kountze if uninsured, on Medicaid or Chips.  Please call before going to see if your child qualifies to receive immunizations there - Phone # 246-5188. Monday & Tuesday from 7-11am & 1-4pm, Wednesday & Thursday 8-11am and 1-5pm; and Friday 8-11am. 

 

  • Beaumont Health Department on Washington Blvd. in Beaumont only if uninsured. Please call before going to see if your child qualifies to receive immunizations there - Phone # 832-4000.  Monday through Friday 8-11am & 1– 4pm. You do not have to be a resident of Jefferson County to receive immunizations.

 

  • China-Community Clinic on Railroad Ave in China, if uninsured, or if China Community Clinic is the Primary Care Physician. Please call before going to see if your child qualifies to receive immuni-zations there.  Phone # 752-2301. Monday - Friday from 9am-12:00 & 2-5pm. Take your immunization records with you for their files.

               

Please send the record to school. A copy will be made and the original will be returned to you. The immunization record may also be faxed to the school.  The fax number is 409-752-3623.
